  3. Do you have a Federal Express account number?
  4.     (yes) Type Account Number in space marked, "Senders FedEx Account".
  5.     (no)  One time delivery, no Account Number needed.
  6. {  Type Account Number in space marked, "Date".}
  7. AREA 1, Type your name, company, street address, city, state, and zip.
  8. AREA 2, Type recipients name, telephone number, company, street
  9. address, city, state and zip in the same fashion as above.
  10. AREA 3, Does this shipment have a reference number assigned?
  11.     (yes) Type in "Reference Number" area you number, up to
  12.     24 characters will appear on your bill.
  13.     (no)  You may leave the "Reference Number" space blank.
  14. PAYMENT SECTION, select what type of payment you want and who will
  15. pay for it:
  16.     (Bill Sender.)
  17.     You will pay for shipping charges that will be charged to your
  18.     Federal Express Account.  Mark an X in the appropriate box.
  19.     (Bill Recipients Federal Express Account Number.)
  20.     Charge the recipient the cost and put their Account number on
  21.     the line below.  Mark an X in the appropriate box.
  22.     (Bill 3rd Party Federal Express Account Number.)
  23.     Bill another company other than your company and the recipients
  24.     company, put that account number on the line below.  Mark an X in
  25.     the appropriate box.
  26.     (Bill Credit Card.)
  27.     Bill American Express, Visa, Master Card, or Diners Club, put the
  28.     card number on the line below.  Mark an X in the appropriate box.
  29.     (Cash.)
  30.     You will give the pickup driver the payment or you can drop the
  31.     package off at a Federal Express station and pay there.
  32. AREA 4, Shipping Services:
  33.     (Priority 1.)
  34.     This is overnight delivery using your own packaging.  Mark an X in
  35.     the appropriate box.
  36.     (Courier Pack, Overnight Envelope.)
  37.     This is overnight delivery using the Federal Express envelope,
  38.     holding up to two pounds of printed material.  Mark an X in
  39.     appropriate box.
  40.     (Overnight Box.)
  41.     This is overnight delivery using the Federal Express box.  Mark an
  42.     X in the appropriate box.
  43.     (Overnight Tube.)
  44.     This is overnight delivery in the Federal Express Tube.  Mark an X
  45.     in the appropriate box.
  46.     (Standard Air.)
  47.     This is a two day air delivery service.  This is a cheaper service
  48.     and the package is guaranteed to get to its destination in two
  49.     working days.  This is a good service for local packages, consult
  50.     Federal Express for details.  Mark an X in the appropriate box.
  51.     (Overnight Letter.)
  52.     This is overnight service for printed materials weighing 1/2 pound
  53.     or less.  Mark an X in the appropriate box.

  56.     (yes) Mark an X in the appropriate box.
  57.     (no)  Go to section H on the form and write in the address of the
  58.     Federal Express station in that city.
  59. Do you want Saturday Delivery?
  60.     (yes) There is an extra charge of $10.00.  Mark an X in the
  61.     appropriate box.
  62. Are you sending dangerous goods?
  63.     (yes) Check with Federal Express for a list if what is dangerous.
  64.     There is an extra charge of $5.00.  Mark an X in the
  65.     appropriate box.
