- Do you have a Federal Express account number?
- (yes) Type Account Number in space marked, "Senders FedEx Account".
- (no) One time delivery, no Account Number needed.
- { Type Account Number in space marked, "Date".}
- AREA 1, Type your name, company, street address, city, state, and zip.
- AREA 2, Type recipients name, telephone number, company, street
- address, city, state and zip in the same fashion as above.
- AREA 3, Does this shipment have a reference number assigned?
- (yes) Type in "Reference Number" area you number, up to
- 24 characters will appear on your bill.
- (no) You may leave the "Reference Number" space blank.
- PAYMENT SECTION, select what type of payment you want and who will
- pay for it:
- (Bill Sender.)
- You will pay for shipping charges that will be charged to your
- Federal Express Account. Mark an X in the appropriate box.
- (Bill Recipients Federal Express Account Number.)
- Charge the recipient the cost and put their Account number on
- the line below. Mark an X in the appropriate box.
- (Bill 3rd Party Federal Express Account Number.)
- Bill another company other than your company and the recipients
- company, put that account number on the line below. Mark an X in
- the appropriate box.
- (Bill Credit Card.)
- Bill American Express, Visa, Master Card, or Diners Club, put the
- card number on the line below. Mark an X in the appropriate box.
- (Cash.)
- You will give the pickup driver the payment or you can drop the
- package off at a Federal Express station and pay there.
- AREA 4, Shipping Services:
- (Priority 1.)
- This is overnight delivery using your own packaging. Mark an X in
- the appropriate box.
- (Courier Pack, Overnight Envelope.)
- This is overnight delivery using the Federal Express envelope,
- holding up to two pounds of printed material. Mark an X in
- appropriate box.
- (Overnight Box.)
- This is overnight delivery using the Federal Express box. Mark an
- X in the appropriate box.
- (Overnight Tube.)
- This is overnight delivery in the Federal Express Tube. Mark an X
- in the appropriate box.
- (Standard Air.)
- This is a two day air delivery service. This is a cheaper service
- and the package is guaranteed to get to its destination in two
- working days. This is a good service for local packages, consult
- Federal Express for details. Mark an X in the appropriate box.
- (Overnight Letter.)
- This is overnight service for printed materials weighing 1/2 pound
- or less. Mark an X in the appropriate box.

- Hold the item in the Federal Express station?
- (yes) Mark an X in the appropriate box.
- (no) Go to section H on the form and write in the address of the
- Federal Express station in that city.
- Do you want Saturday Delivery?
- (yes) There is an extra charge of $10.00. Mark an X in the
- appropriate box.
- Are you sending dangerous goods?
- (yes) Check with Federal Express for a list if what is dangerous.
- There is an extra charge of $5.00. Mark an X in the
- appropriate box.
